Contact form delivery
Contact enquiries are sent to Web3Forms for processing and delivery to FASMI. This includes the name, email address, subject and message entered in the form. Web3Forms and its service providers may process and store submissions outside Canada under their privacy practices. Please do not submit medical records, detailed personal health information or financial information through this form.
